Please use this Form to cancel appointments more than 24 hours in advance. If you need to cancel at short notice, please ring the surgery. If you are unable to attend an appointment please let us know in plenty of time to assign your appointment time to another patient.
You can also cancel your appointment by calling the Practice on 0191 3700500 and choosing option 4. This is a voicemail service so you will not need to wait in a queue.
