New Patient Registration

In order to register with our Practice you will need to produce two forms of identification, one photographic and another with proof of address.

We welcome new patients who live within our Practice area.  As it often takes some time for your medical records to be forwarded from your former Practice, all newly registered patients will be asked to complete a health questionnaire and are invited to use our Surgery Pod to have a new patient health check.  Our Reception staff are fully trained in the use of our Pod and are happy to assist you to complete your check. 

The health check will consist of checking weight, height and blood pressure, as well as dietary habits, smoking status, alcohol intake, any medication you may be taking and relevant family medical history.

If you wish to give someone else (eg a spouse) authority to contact our Practice on your behalf to discuss your medical care, you need to complete a Third Party Consent form, which is available from Reception.

SystmOnline Services

We offer patients the ability of making and cancelling appointments, ordering repeat medication and viewing their online detail read coded data.  If this is something that you would be interested in, please enquire at Reception.  However, please note that you must provide two forms of ID, one photographic and one with proof of address.
